Many health insurance companies include this program in their benefits. To see if yours does:
- Call the phone number on the back of your insurance card and ask about VillageHealth.
- Ask your kidney doctor if it is an option for you.
- Look at your insurance plan to see if it is a part of your benefits.
If you have a chronic (long-lasting) health condition and health insurance, you may be eligible for VillageHealth.
In your first call, you will talk to an engagement specialist who will explain the program and get to know you and your health goals. We want to understand your needs and how we can help.
After the call, we will:
- Work with your doctors to learn more about your health and care plan.
- Follow up with you to make sure you get what you need.
No. If VillageHealth is a part of your health insurance, you will not pay anything.
The VillageHealth Portal is your 24/7 resource that gives you access to education related to your care, recipes and more. You can also see if you have any upcoming telehealth appointments and easily complete health forms.
You may have access depending on your health insurance provider or doctor. Some doctors partner with us to support their care plan, and we also work with some insurers to provide you with extra support. To find out if you’re eligible, visit the Find Us page or contact your insurance.
- Visit portal.villagehealth.com and click “Create Account.”
- Create a username and password and enter your name, last 4 digits of your social, and contact information.
- Press “Finished.”
If you are having issues registering for an account or accessing the portal, please contact technical support at 833-818-1731.
You can think of the VillageHealth Portal as your support hub. It has extra resources like education, recipes, a meal planner, health tips and more. This portal is meant to work alongside your doctor’s portal by providing easy access to content that may be meaningful for you. It does not replace your doctor’s portal. For your lab results or notes from your doctor, you can still go to your doctor’s portal to find health information related to your primary care or other specialist visits.
If you are an authorized caregiver who needs help accessing a family member’s account, you will need to contact that family member to get access. To get set up, you can also create an account on behalf of your patient.
No. Using the VillageHealth benefit will not change:
- The doctors you see
- The care you get
- Your insurance benefits
- Your benefits in the future
